At first glance, waking up every night between 2 and 4 a.m. looks like a sleep problem.
But when you zoom out, a different picture often begins to emerge.

Afternoon energy crashes.

Perimenopause and Hashimoto's each affect many of these systems independently. When they occur together, it's understandable that sleep disruption may feel more persistent or more difficult to attribute to a single cause.

Perimenopause Factors
During perimenopause, estrogen and progesterone fluctuate—often dramatically.
Progesterone has a naturally calming effect on the brain and nervous system. As it declines, many women notice lighter sleep, increased nighttime awakenings, or difficulty falling back asleep.
Estrogen also plays an important role in regulating body temperature. As levels fluctuate, hot flashes, night sweats, and subtle changes in temperature regulation can interrupt sleep, even if you don't fully wake up.
At the same time, changes in cortisol timing can cause your body's natural wake-up signal to arrive too early, leading to those frustrating early morning awakenings.
Hashimoto's Factors
Hashimoto's can affect sleep in more ways than many women realize.
While thyroid hormone changes certainly influence energy levels, Hashimoto's can also affect several systems that support healthy sleep.
These include:
Chronic low-grade inflammation that may interfere with restorative sleep.
Immune system activation that can leave you feeling "wired but tired."
Changes in thyroid hormone levels that influence metabolism and circadian rhythm.
Blood sugar instability, which can trigger overnight cortisol and adrenaline release.

Is accompanied by loud snoring, gasping, or breathing pauses.

Leaves you feeling exhausted despite spending enough time in bed.
When you do meet with your provider, bring observations rather than conclusions.
Before your appointment, consider tracking:
What time you typically wake.
Whether you wake feeling hot, anxious, hungry, or wide awake.
How rested you feel in the morning.
Changes in your menstrual cycle or menopausal symptoms.
Whether stress, exercise, meals, or alcohol seem to influence your sleep.
Any changes in your thyroid symptoms or medications.
These observations can help your healthcare provider see a more complete picture.
How to Talk About It with Your Doctor
The most powerful tool you bring to any appointment isn't a perfect vocabulary—it's a clear understanding of your own patterns.
Start with the experience:
“I’ve been waking up between 2–4 AM and have trouble falling back asleep.”
Then add patterns:
“It happens most nights, and I’ve noticed changes in my stress levels and energy.”
Then invite exploration:
"Could my sleep changes be related to both perimenopause and my thyroid health?
Consider asking follow-up questions like:

Would additional testing be appropriate based on my symptoms?
What lifestyle strategies do you recommend alongside medical treatment?

Support Stable Blood Sugar
Aim for balanced meals that include protein, healthy fats, and fiber throughout the day. Some women find that avoiding long gaps between dinner and bedtime helps reduce overnight awakenings.
Calm Your Nervous System
Gentle breathing exercises, stretching, meditation, journaling, or a relaxing evening routine can help signal safety to your nervous system before bed.
Create a Cooler Sleep Environment
Keep your bedroom cool, choose breathable bedding, and limit alcohol or heavy meals close to bedtime if they tend to worsen nighttime awakenings.
Anchor Your Circadian Rhythm
Morning sunlight, consistent wake times, and limiting bright screens before bed all help reinforce your body's natural sleep-wake rhythm.
Review Nutrient Status
